Support Group -

Mentis Assist has worked with people living on the South Eastern/ Outer Eastern / Bayside region experiencing mental illness and their families for over 30 years under a range of community based mental health support programs. We understand the diverse requirements of the target group in the Casey catchment and as such have provided a range of programs to flexibly meet the needs

Our typical clients have the following characteristics: Long term homelessness , Substance use disorder , Chronic and untreated health conditions , Repeated presentations to emergency departments , Forensic histories and No family or social supports. We provide group, individual, outreach, clinical, non-clinical, structured and evaluated cutting edge rehabilitation programs to people with a severe mental health illness and associated disability In our day to day operations with over 1000 clients, we regularly experience examples of good service and exemplary practice that produce the kind of outcomes that people with psychiatric disabilities as a result of mental health issues and their families desire.

Mentis Assist is a well-established organisation with a track record of good governance and effective management, supported by robust systems and skilled leadership, with a strong focus on client and carer participation in service design and review Mentis Assist provides recovery-focused individual and group support In 2018 we provided services to over 1000 clients Mentis Assist Support staff provide wrap around care that is individually tailored to the person’s needs by coordinating formal/informal supports and services This is achieved by working in collaboration with Monash Health and other allied health and specialist service providers.

Project Summary:

Mentis Assist will provide a safe space for people from the City of Casey with a severe and enduring mental illness who are wanting to have a specified activity in order to assist with their rehabilitation. We intend to run two full days of groups for up to ten people at a time. We would support adults who have a diagnosed psychiatric condition in conjunction with impairments that affect their capacity for social and economic participation. Mentis Assist has existing streamlined referral pathways that provides easier access for mutual clients with Monash Health. Where clients cannot be allocated to a service immediately, they will be clinically prioritised in accordance with the DHHS 'Statewide mental health triage scale guidelines'.

Groups already run in other municipalities by Mentis Assist in 2019 include the following - Substance Abuse/Harm minimisation (self-management, service provider linkages) - Voice hearers groups (self-management, de-stigmatising) - Young people's groups (psycho-social education, self-management strategies, de-stigmatising, connecting with others) - Hepatitis C support - Optimal Health (self-management) - Exploring educational, employment and/or volunteering opportunities within the local community - Independent Living - developing the skills and knowledge needed to obtain and maintain appropriate and affordable private rental properties.

We are passionate about working with people experiencing mental ill health to gain access to timely, quality supports to provide opportunities to enjoy a meaningful life. We use these supports to strengthen self-identity, personal responsibility and hope. Mentis Assist prides itself in having a credible local presence. Over

Mentis Assist believes that we have the right mix of experience, staffing, knowledge of the local community, policies and procedures, quality systems and wrap around services to provide safe secure meaningful supports to clients participating in this exciting project.

Project Outcomes:

Mentis Assist is committed to ensuring it supports client recovery and recognises that this will vary in how ‘recovery’ manifests. For each client the recovery journey will be shared by the client with whom he/she has varying relationships (family, neighbours and/or friends), at different stages in their journey, and these people will support and be involved in different ways.
For many, people recovery will be a lifelong journey. There will also be some people for whom ‘recovery’ is a destination for which they are yet to identify. For these people meeting the daily challenges of life will be their focus. The group program will access each clients progress every six weeks through the validated "Star outcome measurement".

At Mentis Assist our service model recognises that regardless of where a client is on their recovery journey, our service must ensure what we do is purposeful, professional and effective. This means there will be a big emphasis on assessment, planning, service coordination, targeted case work and review/discharge.
The group model we propose here provides an integrated approach across key sectors including state and commonwealth funded mental health programs, community mental health, homeless and housing, public and private mental health services, primary health care, drug and alcohol, disability and Aboriginal health care and allied services.

We regularly implement client satisfaction surveys for accreditation and have safe established systems for collection, storage and analysis
